With a capacity of around 400 beds, Peacehaven Nursing Home provides compassionate, individualized, holistic care to all without discrimination.
We offer a comprehensive gamut of services to care for the sick, frail and elderly who have physical or mental disabilities with mínimal or no appropriate caregiver support at home.
The Home has 14 separate Resident Living Areas (RLAs), each designed as an individual home-like environment. Hope RLA is specially designed to meet the needs of relatively independent residents with dementia.
Peacehaven Nursing Home has also been awarded the bizSAFE level 3 status which affirms our commitment to safety and health standards at the workplace.

RESPONSIBILITIES
About the Job
The Programme Executive (Eldercare) will assist the Centre Manager to drive the eldercare programmes and community outreach.
Responsibilities:

Centre Engagement
- Alignment with organisation and department vision and care philosophy
- Perform community screening, befriend the elderlies, and support in outreach activities or public education
- Conduct health assessments for seniors/clients
- Establish and maintain good interpersonal relationships with clients, caregivers, volunteers and the community
- Ensure the safety and wellbeing of clients, implement and monitor progress of clients' individual care plan
Volunteer Management
- Assist in volunteer management and conduct orientation programme
- Train volunteers in conducting active ageing activities and befriending for seniors/clients
Programme Planning and Execution
- Plan, organise, implement, monitor and evaluate the Centre programmes, activities and events and work out a regular activity calendar
- Assist in sourcing and creation of activities to improve and refresh resources or activities
- Assist and facilitate activities for clients together with the care team
- Facilitate or conduct classes for senior lifelong learning
Staff Relations
- Work closely with the care team to display team spirit and ensure cohesive work relationships for the best interest of our clients
Other Duties
- Participate and receive relevant training to excel in the job functions
- Any adhoc duties assigned by Supervisor
QUALIFICATIONS
ITE/Diploma in relevant disciplines such as Community Care and Social Service, Gerontology, or other relevant fields in Social Sciences
Job Requirements
- 23 years of relevant working experience, preferably in a eldercare programmes and community outreach or have volunteered previously with seniors in a community setting
- Excellent interpersonal and communication skills, especially with seniors
- Proficient in IT
- Able to converse with elderly in English, Mandarin and/or dialects (in order to handle queries and concerns from English, Mandarin and/or dialects speaking patient and/or clients)
- Resourceful, proactive, responsible and clientcentric
- Ability to multitask and meet deadlines in a fastpaced environment
- Team player with strong planning and organising skills
