We are expanding

Looking for a Medical Social Worker for our new nursing home at
Macpherson , to commence work around or after
April 2023 .

Duties & Responsibilities:

Assess the strength and resilience of the resident, families, and social support systems to help the individual function within the community
Coordinate social services through a case management approach
Identify and refer cases for financial assistance
Address psychosocial needs through care and counselling
Educate resident's family member on the physical and psychosocial needs and mediate familial conflicts
Assist staff members in understanding the significant social and emotional factors related to the residents' health problems
Assess risk of selfharm and to others
Work with nursing and other allied health professionals, residents and their families to develop and implement the care plan and the post discharge care plan
Discuss longterm care issues with the resident and family
Train other staff members on communication skills, mediation skills, stress management etc
Other duties assigned by the Management

Requirements:

Degree/Post Graduate Diploma in Social Work with relevant experience
Minimum 3 years of relevant experience
Registered Social Worker with Singapore Association of Social Workers
Good interpersonal and communication skills

Salary will commensurate with relevant working experience and qualification.
