Job Responsibilities:
1) Lead in developing strategy of partnership with social care providers, grassroots organisations
2) and community stakeholders on population health

3) Build strategic partnership with social care providers, grassroots organisations and community stakeholders

4) Lead team in developing a network of volunteers to support population work

5) Collaborate with social care providers and community stakeholders on population health projects and social health integration

Job Requirements

Bachelor Degree in any related discipline, with healthcare, corporate planning/strategic planning and partnership development related experience.
At least 10 years relevant experience, with minimum 5 years of supervisory or leadership experience.
