Case management to work on client’s issues or counselling when required, being the information and referral (I&R) duty officer for certain duties in each month, conducting intake sessions for walk ins or referrals, liaising with other agencies and stakeholders in the community, advocating for clients who face barriers from the systems, empowering clients to be independent and to work towards meeting their goals and ultimately case closure, conducting home visits to engage clients or assess their home environment, doing up case recordings and assessment case plan (ACP) for each case within the given deadline, doing up social reports for clients when necessary, managing the family bonding event (Family Connectors) for the FSC, working with colleagues from other FSCs to run the family bonding event online and in person for different months and conducting outreach to rental blocks to increase the community’s awareness of the services of the FSC.
