RE: Senior/Medical Social Worker Position 

I understand that you are hiring a Senior/Medical Social Worker (MSW) at your hospital and would like to be considered for the position. I attach my resume for your reference.

I was previously a MSW with Tan Tock Seng Hospital (TTSH) where I developed skills in providing patient- and family-centered care at the TTSH Rehabilitation Centre, serving patients who have experienced stroke, traumatic brain injury, spinal cord injury and orthopaedic injury. I also facilitated discharge planning including referrals to community hospitals and nursing homes. In addition, I was also trained in Advanced Care Planning (ACP) and I facilitated ACP discussions with patients and families when needed. 

Furthermore, I gained experience in serving elderly patients at Hospice Care Association (HCA), supporting patients at the end-of-life and their families. I have also worked at the National University Hospital (NUH) as a Care Coordinator for a pilot programme supporting youth with chronic illness. In addition, I completed a year-long training and internship at a developmental disabilities clinic during my Masters programme in Social Work. 

I left TTSH and NUH to care for my children at home, as well as for my elderly aunt at the end of life. I feel that my professional and personal experiences will enable me to perform the role of Senior/MSW at your hospital.  

My last salary was $3,780 (TTSH, full time) / $1,800 (NUH, part time). Please consider providing remuneration of $4,000/month full-time should you hire me for this position. I can also consider a part-time position if available.

Experience

I obtained a Masters in Social Work from the University of Wisconsin-Madison (USA) in May 2011. During my Masters programme, I was also a trainee in the Maternal and Child Health - Leadership Education in Neurodevelopmental and Related Disabilities Programme (MCH-LEND) and completed a year-long internship at a developmental disabilities clinic. I also completed a second internship at the University of Wisconsin-Madison Hospital Neurology Unit.
