Provides comprehensive care for clients with complex health needs following nursing practice standards.

  • Conducts teaching and counseling sessions for clients and their families.
  • Maintains high standards of care management through assessment, planning, implementation, and evaluation.
  • Ensures adherence to established care standards and practices.
  • Updates and maintains Standard of Practices for guiding nursing procedures.
  • Guides staff in direct client care to maintain compliance with standards.
  • Conducts nursing audits and addresses nonadherence issues.
  • Oversees staff development and competency enhancement.
  • Evaluates community nurses' performance, offers feedback, and contributes to professional development.
  • Facilitates orientation and induction programs for new staff.
  • Supports the Assistant Director of Nursing and manages nursing operations in their absence.
  • Acts as a resource for clinical nursing and training matters.
  • Promotes evidencebased practices in care delivery.
  • Conducts quality improvement projects and contributes to performance appraisals

Qualifications:

  • Registered Nurse with the Singapore Nursing Board.
  • Degree or Diploma in Nursing.
  • Advanced/Specialist Diploma in Geriatric/Community Nursing is an added advantage.
  • Minimum of 8 years of relevant hospital work/community experiences.
