The jobholder is responsible to provide a patient-centric, personalised, specialised and compassionate care to home care clients' needs, in accordance with established policies and procedures, standards of nursing care and practices.


Clinical and Psycho-social Aspect:


  • Responsible for the provision and maintenance of quality and safe clinical services to the homecare clients which includes the following:
  • Conduct initial clinical assessment of new clients
  • Conduct 6 monthly review of existing clients and, as and when required when condition changes.
  • Formulate individualised and holistic care plan, and review whenever required.
  • Ensure proper and accurate nursing documentation.
  • Perform nursing procedures in accordance with the core competencies as a Registered Nurse as specified by Singapore Nursing Board.
  • Work closely with multidisciplinary team comprising of Allied Health Professionals, Registered Medical Practitioner, etc.
  • Liaise and communicate directly with clients and sponsors regarding any issues that may arise and provide solutions.
  • Provide psychosocial support, such as establishing effective communication and understanding their psychosocial needs, etc.
  • Liaise with clients and other stakeholders to refer to other relevant services, such as counselling, occupational therapy, dietetic, etc.

Administrative and Operations Support

  • Assist Senior Staff Nurse/Homecare Manger in supporting the daily operations of the home care department which includes the following:
  • Handling home care enquiries and provide financial counselling when necessary.
  • Providing administrative support, managing feedback, complaints and identifying situations where clients require additional attention, etc.
  • Collaborating with other related departments, such as IT, finance, etc., to ensure smooth operational flow of the Home Care business.
  • Coordinating clients' care arrangements, such as rostering, rotating 24 hours oncall basis for emergencies, etc.
  • Achieve the set targets in meeting the profitability of the department.

Manpower Development and Support

  • Provide training and supervision to the team of care staff, when required.
  • Plan and coordinate the assignments of care staff according to their job scope and competencies based on client's care needs.
  • Assist to manage a team of care staff and monitor their performances and developments.
  • Assist to oversee the wellbeing of care staff and manage any staff issues, when required, eg. conflict management, clients' feedback about staff, etc.

Quality Service and Incident Management

  • Lead quality improvement projects and initiatives
  • Identify risks, mitigate and perform service recovery
  • Conduct relevant audits in areas of job scope
  • Manage and report any incident, and escalate when management decision is required

Other Duties

  • Perform adhoc duties and responsibilities, as assigned by Homecare Head
  • Cover duties of care coordinator, during his/her absence, if necessary

Qualifications:


  • Possess a Bachelor Degree in related field and/or Degree or Advanced Diploma or Diploma in Nursing will be an added advantage
  • State Registered Nurse with valid practicing license issued by the Singapore Nursing Board
  • Valid certification in Basic Cardiac Life Support (BCLS)
  • At least 8 years' experience in management of department and have provide supervision to a team of subordinates
