Roles & Responsibilities

Job Description

• Provides home nursing care.
• Makes the initial home visit and performs a biopsychosocial and environmental assessment and conducts subsequent visits.
• Develops a nursing care plan and implements nursing interventions as outlined in the care plan.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Provides nursing services requiring substantial and specialized nursing skill.
• Provides patient and caregiver education.
• Prepares clinical documentation.
• Co-ordinates with other team members and updates them of changes in the patient's status and needs.
• Evaluates the patient's response to care interventions on a regular basis and recommends revision of care plans as necessary.
• Functions as the primary care manager for a given patient, where appropriate.
• Assists the nurse-in-charge with stock inventory.

Requirements

• Registered Nurse with Singapore Nursing Board
• Basic computer skills for electronic health record documentation
• Good communication & interpersonal skills
• Good team player
