- Provides home nursing care.
- Makes the initial home visit and performs a biopsychosocial and environmental assessment and conducts subsequent visits.
- Develops a nursing care plan and implements nursing interventions as outlined in the care plan.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Provides nursing services requiring substantial and specialized nursing skill.
- Provides patient and caregiver education.
- Prepares clinical documentation.
- Co-ordinates with other team members and updates them of changes in the patient's status and needs.
- Evaluates the patient's response to care interventions on a regular basis and recommends revision of care plans as necessary.
- Functions as the primary care manager for a given patient, where appropriate.
- Assists the nurse-in-charge with stock inventory.
- Registered Nurse with Singapore Nursing Board
- Basic computer skills for electronic health record documentation
- Good communication & interpersonal skills
- Good team player
