    Roles & Responsibilities

    Job Summary:

    This Programme Executive position requires the candidate to proactively reaching out to clients, organize social activities and enabling clients to adopt an active lifestyle and pre-empt social isolation. The nature of the job also requires to refer clients to appropriate community care and social assistance when necessary. With Age Well SG initiatives, you shall work with primary care physicians (e.g. family doctor) to empower residents to chart their own journey towards healthy and active lifestyles.

    Key Responsibilities:

    • Plan and conduct Evidence-Based active ageing programmes.
    • Continuous Improvement in providing Quality Services and Quality Programmes
    • Administering new intake and provide necessary health assessments for new clients
    • Recruit, train and match volunteers to appropriate services.
    • Plan and Coordinate support for clients who requires Befriending and Buddying services.
    • Lead group work and outreach activities with the AAC team.
    • Build community partnerships and enhance existing resources.
    • Support AAC-CREST integration work.
    • Perform administrative work including maintaining up-to-date records of clients and volunteers.
    • Attend to enquires from clients.
    • Maintain a database of membership and volunteer management using the in-house AAC client management system.
    • Taking and monitoring vital sign for clients
    • Any other duties as assigned by Manager/Executive Director

    Requirements:

    • Prior working experience in Senior Activity Centre, Active Aging Centre would be advantageous.
    • Resourceful, proactive, responsible, and focused on clients
    • Good communication & interpersonal skills. Able to converse in local dialects will be an added advantage.
    • Strong passion and desire to work with seniors.
    • Strong knowledge of using MS office.
    • Proficient in google suite applications.
    • Able to work independently.
    • A team player.

    Qualifications:

    Diploma in Social Work and any relevant training certificate
