The Assistant Manager will support in the work of facilitating, enabling and driving integrated care across NHG - covering Central & North zones, with focus on setting up and managing the funding and commissioning framework to develop new care models for the integration of care.
Hence, he or she is expected to be a team player, working closely with various clinical steering committees and administrative units in NHG HQ as well as Integrated Care Networks.
Major Duties & Responsibilities
- Supports the coordination and integration of new and existing clinical programmes into integrated care plans and services and standardised care protocols, with the intention to evolve into mainstream bundled care plans/models across NHG population health strategic priorities and River of Life framework.
- Collaborate and provide support for stakeholders in various Clinical Steering Committees' and Workgroups in the development of strategic Population Health programmes roadmap so that integrative care work planning, prioritisation and implementation timelines are enabled for success.
- Lead the ecosystem mapping with the Clinical Steering Committees to identify current services or gaps and support the framing, strategic planning, and resourcing of integrated care programmes in tandem with the respective Chairs of the clinical Steering Committees and Workgroups.
- Drive the Programme Management and monitoring support with dashboard visualisation reporting to build data analytics capability in evaluating program effectiveness. Track and report on program progress, including key performance indicators and outcomes to ensure successful program execution.
- Provide secretariat and administrative support to NHG Committees/Workgroups and GIC meetings which includes curating agenda with the Chairperson and minutes taking.
- Undertake any other tasks or adhoc duties as assigned by the Supervisor
Job Requirements
- A Bachelor degree from a recognised university with at least 23 years of working experience.
- Experience in the social care or healthcare industry will be an advantage.
- Experience in lead/colead facilitation of discussion in diverse groups
- Ability to manage projects
- Strong written, analytical and communication skills
- Good PowerPoint and presentation skills.
- Good communication and interpersonal skills necessary for engagement of different stakeholders within the regional health system.
- Independent, analytical, resourceful and organised.
- Able to multitask, flexible, diplomatic and adapt to a fastpaced working environment.
