    Summary:
    The core function of the position is to provide physiotherapy services to frail elderly in their home. The job holder also leads the effort to put in place a framework for fitness and functional training

    within the community wellness hub


    Principal Responsibilities and Duties:
    Community Wellness Hub-Gym programme

    Work with head of department to develop framework, course structure for fitness and functional training.

    Identify community needs and designing programmes to meet new needs

    Evaluate and identify gaps in services to develop strategies for service improvement


    Establish necessary policy and work processes to ensure efficient delivery of the service and the safety of clients and staff.

    Ensure that staff involved in the delivery of fitness and functional training are trained and equipped.

    Support Community Enablement Programme by organizing and conducting talks/activities.

    Day Rehabilitation Centre

    Work with client and/or caregiver to assess and monitor client's treatment progress

    Ensure accurate and timely documentation of client's condition according to MOH service requirement.


    Liaise with other internal and external healthcare or non healthcare workers caring for the same patient to plan, execute and appraise patient care plan and progress.


    Provide or activate care giver training to patients/caregivers and evaluate the effectiveness of the programme by assessing the improvement in care and caregiver/s coping response.

    Train and mentor junior therapists, therapy assistant and student.

    Improve existing workflow, processes, structures for home and centre-based services to increase productivity

    Adhere to code of professional conduct according to Allied Health Professional Guidelines.

    Participate in professional development activities and projects (e.g training, quality improvement project, research or any new initiatives as assigned


    Essential Skills and Qualifications:
    Degree in Physiotherapy.

    At least 5 years of experience providing physiotherapy to the elderly.

    Strong knowledge of community engagement and needs assessment

    Familiar with the health care sector, key stakeholders in the sector and resources available.
