    Roles & Responsibilities

    Purpose

    To effectively manage cases receiving foster care services and provide social workers with professional supervision in case management and group work.

    Responsibilities


    • Conduct in-depth assessment and prepare Foster Parent Assessment Reports, presenting findings at the panel and following up on decisions and recommendations


    • Supervise and support foster placement so as to prevent breakdowns in existing care arrangements


    • Conduct visits to ensure that the placement is able to safeguard and promote the safety, stability and well-being of the child


    • Spearhead case movement through team meetings, monthly case review sessions, COF, CART reviews etc


    • Identify at-risk situations and develop strategies and intervention plan to address child safety issues, and well-being


    • Provide a 24-hour hotline for foster parents per duty schedule and ensure that the team follows up on outcomes


    • Monitor and evaluate the effectiveness of case management and other support programmes and activities extended to foster families


    • Assess case progress and programme outcome and effectiveness


    • Ensure, support and monitor SOPs and participate in audit processes


    • Lead and facilitate meetings and case conferences


    • Look into best practices and implement them within the department


    • Provide guidance and supervision for social workers and case workers


    • Conduct regular case discussions with social workers and supervise their case management


    • Monitor caseloads and performance of social workers


    • Support or conduct practice research


    • Cover for Head/Fostering Services whenever necessary


    • Participate in road shows and other community outreach events as and when necessary.


    • Organise and participate in inter-agency efforts in support of the welfare and development of the foster child

    Requirements

    · Bachelor of Social Work / Masters in Social Work

    · At least 5 years of relevant experience with 2 years in a supervisory position

    · Experience in working with children and young persons & their families
