Job Responsibilities

• Develop and implement strategy/plan to develop the healthy precincts under your care
• Identify internal and external partners and platforms for the development of healthy precincts, and engage and collaborate with them to enable successful on-ground activation
• Develop and implement community health plan for the precincts under your care, with emphasis on preventive health and social-health integration
• Track and review progress on implementation of community health plan for different focus areas
• Secretariat work for meetings
• Any other duties, projects or assignments by Reporting Officer and/or HOD

Requirements

• Bachelor degree in any discipline
• Minimum 2 years of working experience as a project team lead preferably in Healthcare
• Knowledge and experience in planning, programme development, stakeholder engagement and management, and project management
• Able to think critically and strategically, with strong conceptual and analytical abilities
• Strong planning, project management and organisational skills
• Adept in stakeholder engagement and management to foster buy-in and collaboration
• Strong written and verbal communication abilities
• Resourcefulness in problem solving and resilient in overcoming challenges
• Ability to work independently and in a team
• Ability to work effectively under tight deadlines
• Knowledge of the Singapore healthcare system and/or population health models will be advantageous
