Job Requirements:


  • Advanced / Higher / Certificate in Community & Social Services
  • Minimum GCE 'O' Level or relevant certificate
  • Comfortable with working with seniors
  • Mandarin Speaking
  • Singaporean or P R are welcome to apply
  • Outreach and conduct engagement efforts and home visits, if necessary, to seniors in the identified clusters to support and facilitate ageinginplace
  • Coordinate with Volunteer Management team to ensure that engagement sessions are
conducted between seniors and volunteers

  • Manage queries, provide information and referrals to services
Active Ageing Programme

  • Assist in the planning and executing of programmes and activities to ensure seniors are engaged, based on areas of interest of the senior population in the assigned boundary
  • Assist in the coordination and supervision of services and activities for the seniors
  • Assist in the formation of mutual help groups amongst the seniors
  • Maintain network of resources (including community partners) to facilitate ageinginplace for the seniors
  • Assist in evaluation of existing programmes to ensure content and objectives are align with the national initiatives and direction
Befriending

  • Monitor engagement to seniors made through the visit reports by staff and volunteers to prevent lapse in services
  • Conduct assessment and review of seniors' care plan under the programme and referrals made by other community partners
Tembusu Active Ageing Centre and Tembusu Eldercare Centre are located at Eunos Crescent.
