Description

Job Scope:

Responsible to facilitate and drive Advance Care Planning for targeted group of patients
Engage all suitable referrals in Advance Care Planning discussion
Ensure effective communication of patients' Advance Care Plans to different stakeholders (Eg: Allied health professionals, patients' family members etc)
Follow up and review outcome of Advance Care Planning
Follow up on concerns raised during Advance Care Planning discussion
Assist to raise awareness of Advance Care Planning and conduct Advance Care Planning advocacy and facilitator training

Job Requirements:

Degree in Nursing, Social Work, Psychology, Allied Health or equivalent
Good interpersonal and communication skills
Proficient in using MS office
Knowledge in statistics and familiarity with relevant community services will be an advantage.
