Overview


A digital health coach uses technology such a digital data platforms, mobile apps, text messaging and phone calls to provide guidance and support to participants looking to improve their health and well being.

The helap individuals set and achieve goals related to wellness and lifestyle factors. They also provide education and resources to help participants manage chronic conditions. A new preventive health programme featuring digital health coaching has been introduced by Alexandra Hospital.


With the rise of technology and the demand for digital health solutions, digital health coaching has become an attractive and interesting for job seekers.


A digital health coach is a professional who helps patients achieve their health goals by monitoring patients' wellness digitally and creating customised lifestyle and nutritional changes through a care plan.

As a digital health coach, this role have the opportunity to help patients make lasting lifestyle changes while also making a meaning impact on their lives


Responsibilities

  • Conduct initial consultation with participants using data gathered from a digital health platform
  • Create and implement personalized health plans that are aligned with lifestyle goal(s) set by General Practitioner (GP) utilizing digital tools and resources
  • Use data and analytics to track participant progress and adjust plans as needed
  • Monitor participant adherence to the program and encourage continued participation
  • Provide ongoing support and guidance to participants through virtual coaching sessions and personalized nudges
  • Communicate effectively with participants via phone and nudges
  • Maintain accurate and confidential client records
  • Consolidate and routinely update the list of existing activities available within Queenstown community and amongst partners
  • Stay uptodate with the latest digital health technologies and trends

Requirements:

years of experience in healthcare and/or community health setting in Singapore is preferred.

  • Strong teamplayer, with natural ability to interact with patients/ residents, healthcare team and community partners of all levels.
  • Selfmotivated and able to work independently and manage a high volume of clients
  • Organised with strong problem solving and analytical skills
  • Strong communication and interpersonal skills. Ability to use local languages and dialects will be an added advantage
  • Passion for health and wellness and desire to help others improve their wellbeing
