Description
_Participate in case discussions and follow up with the coordination of clinical related issues. _
- _Involve in planning, assessment and monitoring of the programme. _
- _Perform system duties for patient admission, discharge and transfer _
- _Coordinate with multi department to schedule appointments and/or manage bed requests _
- _Coordinate with external vendor to arrange for transport e.g. ambulance and courier of logistics _
- _Manage logistics e.g. consumables & homecare bags and involve in management of stocks _
- _Participate in improving work processes e.g. update of workflows and perform other adhoc responsibilities when required. _
- _At least 1 year relevant experience._
