Your job responsibilities will include but not be limited to the following:
Coordinate health and social care by liaising with various stakeholders within the integrated community care team;Screen residents to determine their health and social care needs;Coach patients and caregivers in managing patients' health and social aspects, including managing chronic diseases, adopting healthier diets, and sharing information on suitable community resources and services for residents;Implement changes to care plans co-developed with residents, in consultation with Senior WBCs and/ or clinical teams; andGuide SAWBCs (Sr Associate Wellbeing Coordinator) & AWBCs (Associate Wellbeing Coordinator) in care coordination & case management duties, patient coaching duties as well as health and social care needs assessments

Job Requirements:
Bachelor Degree in any discipline, preferably in healthcare or sociology discipline;Operational experience in and knowledge of health social and community care sectors preferred;Good verbal and written communication skills; andVersatile to adopt a variety of coaching methods, as suited for different resident and patient profilesCompany information
