You will work closely with our team of healthcare professionals in providing quality clinical care, home care, as well as mobile health screening services.

• The home care nurse's responsibilities include performing patient visits and monitoring their conditions, conducting health screening and nursing care- e.g. administering IV, changing dressings, cleaning wounds.
• Assess and chart observations of the patient's condition at each visit.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Dress or redress wounds and assess the healing progress.
• Traveling to patients' homes and managing their care plans according to physicians' instructions.
• Educating caregivers and family on the aftercare or ongoing care of the patient
• Responsible for the safe delivery of care and clinical decisions for all home care clients.
• Render clinical care and ongoing evaluation of care provision for home care services.
• Providing nursing services at in-house and industrial clinics.
• Maintain proper and accurate nursing documentation for clients' clinical records.
• To carry out their responsibilities in accordance with the Nursing Home Services guidelines, protocols and standards operating procedures (SOPs).
• Any other ad hoc duties.

• Diploma in Nursing or equivalent and recognised by the Singapore Nursing Board.2. Registered with Singapore Nursing Board is preferred.3. Post Basic and Degree in Nursing.4. Experience with advanced wound management is preferred.5. Has a Valid BCLS & AED Cert.6. Able to perform phlebotomy.7. Willing to travel.
