Description
**Job Description: - Conduct casework.- Perform stakeholder engagement and partnerships.
- Perform community development work.
- Design, implement and conduct Groupwork and Interest Based Groups for elderly.
- Provide social and emotional support through home visits, care planning and activity recommendation.
- Provide support for volunteer management.
- Maintain and document records of groupwork and case sessions.
- Provide support to Case Managers.
- Coordinate, plan, develop, implement and evaluate programmes.
- Conduct environmental scans and community need assessments.
Job Requirement:
- Diploma in Social Work
- Experience in eldercare preferred
