    Description
    Roles & Responsibilities
    • Provide Casework and Counselling to teenage girls in a Residential setting
    • Liaise, collaborate with, and provide information to external partners and stakeholders
    • Coordinate and manage all external appointments of the residents
    • Assess risks and needs of residents and their families and formulate the Individual Care Plan (ICP) for each resident accordingly.
    • Ensure execution of ICP's planned action according to goals, in close collaboration with care staff and programs team
    • Provide counselling to resident's family, and conduct restorative work between residents and their families
    • Monitor daily incidents and progress of residents
    • Provide inputs at weekly Disciplinary Committee conferences and ensure execution of outcomes derived from the conferences
    • Ensure and maintain accurate and updated records on each resident
    • Prepare, review and submit all regular and ad-hoc reports required
    • Conduct group work with residents and/or with their families to equip them with the necessary life skills
    • Review relevance of all external proposed classes/workshops/talks that may be useful for residents
    • Coordinate with the respective agencies on the proposed sessions, internal team and Programs department to facilitate sessions
