    [What the role is]

    The Health Promotion Board's (HPB) vision is to build a nation of healthy people. We want to empower residents in Singapore to attain optimal health, increase their quality and years of healthy life, and prevent illnesses, disability and premature death. To achieve this, HPB drives national health promotion and disease prevention programmes, and creates a supportive environment in Singapore where healthy lifestyle options are available and accessible to residents every day.

    To support the Board's goals, the Regional Health Systems and Community Outreach (RHCO) Division collaborates closely with the community and healthcare partners to co-create regional and community-owned solutions to ensure good coverage and sustainable delivery of priority health promotion programmes in the community setting. Key programme areas include active ageing, physical activity, nutrition, substance abuse and mental health. The initiatives cover the general population segments from children to seniors, as well as targeted outreach initiatives to ethnic minority groups and disadvantaged groups (such as those from lower socioeconomic status).

    This person will partner Regional Health System (RHS) in identifying the gaps in services and work processes and standards, and help build providers' capabilities and capacity in the area of preventive health with the support from HPB. He/she will also build partnerships and link up partners into healthy ecosystems so that partners are able to "plug and play" the preventive health bundle programmes in their communities.

    Work scope not limited to:

  • Provide guidance to healthcare cluster on-ground teams to align delivery of programmes and services with national standards in support of Healthier SG
  • Support RHS in Primary Care Providers (PCP) briefing on Preventive Health and Health Promotion Programmes governed by HPB, and train the PCP and their staff to be familarised with functionalities on Healthy 365.
  • Liaise with Population Health Office (MOH and RHSes) to understand their programmes, data collection for the analytics & dashboarding, customer journey mapping and integration to HPB's Preventive Health strategy.
  • Collect, consolidate, and analyse customer experience and service metrics across various outreach and RHS' touchpoints
  • Monitor and identify best practices for internal learnings and applications for areas that are not performing
  • Develop customer journey maps across HPB's and RHS' programmes and target segments to transform clients' experience of health promotion and disease prevention
  • Facilitate on-ground teams to develop and implement action plans to facilitate a holistic customer experience in support of PPH strategy
  • Assist DD/AD in day-to-day functions in implementing, monitoring, analysing datasets and preparing dashboards
  • Monitor if these efforts meet its KPIs and competence requirements, to tailor health promotion for individuals / institutions
  • Requirements

  • Previous working experience as an Outreach and Partnerships Manager for at least 3 years;
  • A very good understanding of partnership development and relationship management. Ability to network easily and build strong collaborations with stakeholders;
  • Excellent communication and interpersonal skills and versatility in dealing with different types of partnerships;
  • Strong organisational and time management skills; good multitasking skills but with attention to details;
  • Ability to work independently, and with teams;
  • Passion to serve, particularly in the community setting
