    Description
    Roles & Responsibilities

    To provide effective and efficient occupational therapy sessions and develop innovative solutions to meet the client needs, in line with best practice. He / She will work with a multidisciplinary team to ensure the coordination, development and delivery of a quality, client-centered therapy service.

    The occupational therapist will also be required to support other departments in providing clinical support where necessary, to ensure a seamless journey for our clients and their caregivers.

    Responsibilities

    Clinical Duties

    • Conduct accurate and holistic occupational therapy specific assessment for clients during admission, periodic care plan reviews and discharge
    • Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making
    • Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders
    • Design plans and conduct occupational therapy interventions for clients
    • Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines
    • Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
    • Recommend appropriate assistive device and equipment as needed
    • Formulate discharge and continuity of care plans based on clients' performance in consultation with relevant stakeholders
    • Facilitate community integration through collaboration with other community partners as required
    • Empower clients and caregivers with methods to enhance health, mobility and functions through self-management strategies
    • Participate in case discussion with the care team using a multidisciplinary, collaborative approach

    Requirements

    • Degree or Diploma in Occupational Therapy
    • AHPC Full Registration
    • Able to converse in English and other languages
    • Minimum 2 years relevant experience preferably with neuro-rehabilitatio
    • Dynamic and proactive personality with good public relation and communication skills
    • Passionate in serving the community
