    Description

    Aged & Disability Support (Community Services & Development)Support the Manager in overseeing compliance in day-to-day operations, directives, KPI and all work processes.

    Conduct home visits, community outreach efforts and conduct community screening for seniors residing in the assigned boundary.
    Develop and implement outreach strategies.
    Build and maintain relationship/ rapport with community seniors.
    Plan and implement activities and programmes based on seniors' needs and interest to promote active lifestyles and social engagement.
    Gather seniors to join the planned programmes.
    Provide support to seniors who requires befriending and track the efforts done.
    Provide information and refer the community seniors to appropriate services as required.
    Assist community seniors on vital signs monitoring.
    Compile and submit required reports on a timely basis.
    Provide administrative support such as putting up document for purchases, claims and processing invoice received.
    Track the expenses for the AAC and keep expenses within budget.
    Supervisory role to associates and ensure their competencies.
    Arrange and track essential courses required.
    Any other duties as assigned by the Manager.
    Attend to walk-in, phone and text enquiries and correspondence.
    Track AAC KPIs such as outreach, programmes participation and befriending.
    Ensure confidentiality of the seniors' data, in line with Organisation's PDPA policyR

    equirements
    :
    A degree in any discipline.3 to 4 years' experience in community care setting is preferred.
    Good communication and interpersonal skills.
    Can-do positive attitude, with interest to engage the seniors.
    Team player who is organized and meticulous at work.
    Eye for details and meticulous in planning of events from start to the endWilling to work on occasional weekends.
    Adaptable to changesAbility to multi-task and perform under pressure.
    Able to work in a fast-paced environment.
    Proficient in Microsoft Office and data presentationCompany information
