- Coordinate care and manage care based on One Care Plan
- Perform assessment of patients' needs, including functional and psycho-social needs and linking up with various resources and community services, based on patient's care plans
- Identify needs and refer patients to community services using AIC's platform, including centre-based services, home-based services and personal care services
- Provide post-hospital discharge follow up calls to patients to ensure resolution of their symptoms, optimisation of care and caregiver competency
- Provide and follow up on patient and family education during telecommunication guided by home care plans and care protocols
- Contact patient's primary physician/care team or ward doctor/nurse to clarify care plans and seek advice on further follow up plans where needed 2. Connect and empower: Engaged, Informed Residents and Carers
- Provide education on chronic disease management and healthy lifestyle to patients and assess their ability to manage their chronic conditions, guided by protocols
- Promote and encourage residents to participate in health and wellness programs that are available in their respective zones eg. health screenings and community service partners
- Guide patient/caregivers in navigating healthcare and community services and increase their awareness of services and facilities near their homes 3. Anticipate care and manage escalations
- Identify early signs of exacerbations of chronic diseases through tele-communications with patients and advise on escalation plans based on patient's care plans
- Provide a /7 tele-triage service to receive incoming calls from patients at times of exacerbations or symptoms development and assess the urgency of conditions and advise on appropriate management including referral to hospital, primary care or other community services.
- Contact Emergency department triage nurse/doctor to expect patient in times of medical emergency and update them on patient's medical condition
