Job summary
The Senior/Staff Nurse, Community Care, RHSO will perform care assessment planning and management for residents according to established protocols and evidence-based practices under appropriate supervision.
S/he will demonstrate competency in care coordination across the healthcare continuum providing holistic care through effective collaboration with interdisciplinary teams and relevant community partners facilitating care transition and coordination for residents based on their individual needs and preferences to encourage self-management continuity of care The Staff Nurse contributes to evidence-based practices innovation quality improvement research projects.
As a Senior / Staff Nurse in Community Care you will be required to perform nursing care health coaching within the areas of Health Promotion & Illness Prevention through patient education Primary Care by performing basic assessment health education to the general public Home Care services for patients transiting from hospital home Any other community related duties will be assigned.

- Possess Diploma Degree in Nursing must be registered with Singapore Nursing Board Minimal 2 – 3 years of relevant work experience in Community Setting Able Able able work independently team Strong organisational coordination interpersonal communication skills Able converse local languages preferred Show more Show less
