Transition Support Officer


RESPONSIBILITIES

  • A Transition Support Officer is responsible for providing handson, practical support, training and guidance to clients who are going through transitions, be it from School to Training & Development Centre, from Training & Development Centre to Employment & Development Centre (or vice versa), from residential to community care (or vice versa).
  • Close collaboration with the multidisciplinary team, consisting of Allied Health Professionals such as the Psychologist, Social Worker, Occupational Therapist, physiotherapist, and / or Speech Language Therapist, in carrying out the Individual Training Plan (ITP), especially in training areas needed to ensure successful transition as according to client's ITP. He or she will also work in collaboration with the Social Workers in supporting the caregivers, in the training and transference of skills into the community or home environment.
  • Areas of work include community mobility skills, community living skills, and home living skills, etc. as outlined in the ITP and recommended by the multidisciplinary team. When appropriate, it will also take into consideration the future care plan of the client in collaboration with the caregivers and the MINDS Future Care Planning Team, as well as community partners & stakeholders such as training providers, community case workers & social workers, training officers & other job coach to ensure successful and sustainable transition for the client.
  • The guiding principle for Transition support should be holistic, person and family Centric as much as possible, keeping in mind the need to trouble shoot issues that arises from integration of client's Individual Transition Plan and actual transition situation, and make adaptations accordingly.

Assess client's readiness for transition and transition training needs in collaboration with the multi-disciplinary -team in MINDS

  • Conduct assessment with regards to clients' transition needs, using a variety of assessment tools, e.g. CAF-R, etc
  • Participate in Transition Planning meetings for clients in collaboration with AHPs and relevant stakeholders
  • To work with Social Workers to engage selected clients and caregivers in discussion on transitions and training needs
  • Document and keep records of client's' transition training needs

Practical hands-on training and support for clients on transition matters

  • Carry out training and support outline on client's transition training needs at the relevant transition settings
  • Work with clients and relevant stakeholders to finetune and adapt training as necessary using holistic, client and family centric approach
  • Training can also include Activities of Daily Living Skills, community living skills independent living skill, behavioural, social and emotional support for clients
  • Document and keep records of client's training progress, adaptations made, etc

Collaboration with multi-disciplinary AHP team, caregivers and community partners including volunteers in advocating for client's transition matters

  • Work with relevant stakeholders to advocate for clients' transition support needs, looking at environmental, social emotional, behavioural support factors
  • Identify areas of continuous learning for clients and support their development as lifelong learners in the relevant environment(s)
  • Connect relevant stakeholders to AHPs for provision of Training and public education, awareness building to increase understanding and awareness of the needs of PWID to ensure successful and sustainable transition for clients in the relevant transition environment(s)
  • Document and keep records of engagements, adaptations made with relevant stakeholders
  • Consolidate good and best practises, knowledge and experience to build a resource bank of best practises and useful tools in collaborative practise, to inform practitioners for replication success, and /or pitfalls to avoid

Review and evaluate the Individual Transition Plan of clients to ensure successful / sustainable transition for clients

  • Conduct reassessment of clients' transition needs, to track progress, i.e. pre and post intervention assessment
  • Revise / adapt transition and training plan if outcome is not met, or to bring clients t
