Job Summary:
The Senior Group Home (SGH) Coordinator will assist in the operations and administration of the SGH; ensure the well-being of the seniors and assist with the office administration.
Job Scope:
- Provide basic monitoring and emotional support to residents of SGH to maintain their overall wellbeing and ensure their safety within the SGH.
- Provide care coordination service and assistance to SGH residents to access social and healthcare services to ensure that their care needs are adequately addressed e.g. meal deliveries, home care, hospital admissions, medical appointments, counselling, etc.
- Maintain and update daily records of clients for adminstrative and intervention purposes.
- Prepare and submit data and reports to Management and relevant authorities.
- Manage the cleanliness and liveability of the SGH.
- Educate and assist the SGH residents to monitor their own health status e.g. medication reminders.
- Assist with the office administration duties.
- Perform such other duties as assigned by the reporting officer.
Job Requirement:
- GCE 'O' Level or Certificate
- Relevant experience in eldercare sector
- Passion to work with seniors
