Job Description:
The incumbent is responsible to support the clinical and administrative operations at Day Care Centre

ESSENTIAL FUNCTIONS & RESPONSIBILITIES

A. CLINICAL FUNCTIONS
Exhibits personal commitment and integrity in the daily care of patients.

Implements instructions given by the Assistant Centre Manager / Centre Manager as part of the multidisciplinary team to achieve the highest level of coordinated care for patients.

Monitors signs, pain and symptoms of health condition; provides prompt feedback to Nursing Aide or the clinical team.

Assists patients with an active lifestyle which may include appropriate physical and social activity for the patients.

Provides a supportive environment for patients to be cared for.

Enables patients to continue with maintenance programmes.

Builds/identifies activities to suit the individual patient or appropriately grouped patients as far as possible, while recognising any relevant group dynamics.

Manages patients challenging behaviours with aim to reduce the frequency, intensity and duration of such behaviours; report to Assistant Centre Manager / Centre Manager should patients refuse to listen.

Ensures the delivery of safe and proper care for the patients, co-workers in the Day Hospice Centre.

Prepares and serves meals; conducts exercises and activities for patients.

B. ADMINISTRATIVE FUNCTIONS
Performs Day Hospice related administrative duties.

Provides regular reports as needed by Assistant Centre Manager / Centre Manager.

Coordinates with peers and volunteers to improve the quality of life for patients.

Participates in departmental and organisational activities and events.

Monitors and maintains the organisation assets.

C. TRAINING AND EDUCATION FUNCTIONS
Initiates and participates in continual learning and development programmes.

Supports the organisation related training and development programmes.

OTHER FUNCTIONS
Performs other duties as required to meet THE ORGANISATIONs vision, mission statements and the organisation

Accompanies patients when travelling in THE ORGANISATION van.

Advises volunteers in safe handling of patients eg. wheelchair pushing.

Identifies opportunities for improvement, develops and implements improvement strategies.

Qualifications
O Level or Certificate in Therapy

Working:
Monday to Thursday 8- 530pm and Friday 8 to 5pm

Salary up to $2400
