Occupational Therapy Student
SPD: adult disability home (Feb - early April 2023)
- Worked in adult disability home (Blue Cross), and Dementia Day Care center.
- Client population includes individuals with intellectual disabilities (mild-moderate), patients with dementia (mild to mod-severe).
- Developed and implemented therapy plans based on client's needs and abilities, resulting in improved leisure participation and social participation skills for individuals with intellectual disabilities.
- Participated in multi-disciplinary meetings (MDM) and contributed to the client's individualised care plan (ICP).
- Performed documentation for the respective client group for both centers which includes progress notes, SOAP notes, and ICP. - Conducted initial assessments (IA) for the respective client group(s).
St Luke’s Hospital: subacute/ community hospital (June - early Aug 2023)
- Client profile: neurological cases (stroke, Parkinson’s Disease), orthopaedic cases (post total hip and total knee replacement) and general deconditioning.
- Conducted initial assessments (IA) on the aforementioned client groups. Developed and implemented occupation based, therapy plans.
- Conducted on-site (home visit) and off-site home assessments (via photos) and recommendations - e.g., grab bar placement via HDB EASE, and equipment prescription such as mobile commode.
- Performed caregiver training and education for the client’s respective caregivers: wheelchair management, transfer training, curb and step management, use of equipment and assistive devices).
- Participated in multi-disciplinary meeting (MDM) to discuss and collaborate on the client’s transition of care plan before discharge.
